Release of Liability:
I hereby certify that the child I’m registering is in my legal custody and has my permission to participate in the courses conducted by ART ZONE Children’s Studio. I further certify that the participant is in good health and has no physical or other impairment which would endanger the participant when engaging in such programs.
In consideration of the services provided by ART ZONE Children’s Studio, I absolve and hold harmless ART ZONE, its employees, officers, or agents from any liability which may result from participation in courses conducted by ART ZONE Child Care.
I understand and acknowledge that ART ZONE Child Care has no obligation to supervise my child at the close of the above activity, and I release ART ZONE, its officers, employees, and agents from any liability resulting from any lack of supervision of my child at the close of the activity.
Tuition Rates and Refunds:
a. Tuition rates are not refundable for most classes unless otherwise stated in writing by ART ZONE Children’s Studio.
b. In the event of extraordinary circumstances, such as a prolonged closure of our facility due to unforeseen circumstances beyond our control, refunds or credits may be issued at the discretion of ART ZONE management.
Missed Classes and Make-Up Sessions:
a. Some missed classes may be eligible for make-up sessions within a six-month window from the date of the missed class. Make-up sessions are subject to availability and must be scheduled in advance.
b. Make-up sessions cannot be guaranteed and are subject to availability of space and resources.
c. Missed classes not made up within the specified time frame will not be refunded or credited.
Food Restrictions and Allergies:
It is the responsibility of the parent/guardian to notify the lead teacher at drop-off if their child has any food restrictions or allergies. This information is crucial for ensuring the safety and well-being of all children under our care.
Photo Release:
Participants involved in ART ZONE programs may be photographed, and such photographs may be used to publicize ART ZONE programs/activities.
Acknowledgment:I have read and understood the terms of this Release of Liability policy and agree to be bound by its provisions. By enrolling my child in ART ZONE Child Care programs, I acknowledge that I have read, understood, and agreed to the terms and conditions outlined in this Release of Liability policy.
ART ZONE Child Care reserves the right to refuse enrollment to any child whose parent/guardian does not agree to abide by the terms of this Release of Liability policy.
